What should a dog grooming intake form include?

At minimum: owner contact and an emergency contact, the pet name, breed, age and weight, vaccination status, any health conditions, allergies or medications, handling and behaviour notes such as biting or sensitivity to nail trims, the service requested, and a signature line. What vaccinations should the record include?

Most salons track rabies and DHPP at minimum, and many also require Bordetella. Requirements vary by location and by your own policy, so confirm what you require and what your local rules mandate.
