Grooming Forms FAQ

What should a dog grooming intake form include?
At minimum: owner contact and an emergency contact, the pet name, breed, age and weight, vaccination status, any health conditions, allergies or medications, handling and behaviour notes such as biting or sensitivity to nail trims, the service requested, and a signature line. The template on this page covers all of it on a single page so you can capture it at the first appointment instead of chasing it later.
Are these grooming forms really free?
Yes. Every template here is free to print, save as a PDF, and use in your grooming business. No signup or payment is required, and you can customize any field to fit your salon.
How do I edit or save the forms?

Do I also need a grooming consent or liability waiver?
Yes. A signed grooming consent and liability release protects you in the event of a nick, a senior-pet incident, or a matting shave-down. The intake form here collects details; build the actual consent form with the free waiver generator.
What vaccinations should the record include?
Most salons track rabies and DHPP at minimum, and many also require Bordetella. Requirements vary by location and by your own policy, so confirm what you require and what your local rules mandate.
